Livewell Southwest Community Nursing Team

MSc Advanced Professional Practice (Community and Primary Care Practitioner) careers

You’ll be able to apply for Band 7 and Band 8 roles, and work with individuals, families and communities within the integrated health agenda. You’ll be competent in complex decision making and contribute to service development through innovation, leadership and research. Your health promotion skills and understanding of the health of populations will be enhanced.

As a community and primary care practitioner, you’ll work as a specialist caring for patients presenting with a range of conditions including urgent and acute episodes, and long-term chronic conditions. Your work will involve both individualised care and care co-ordination in collaboration with other healthcare professionals. Your work may contribute to keeping people out of hospitals or referring patients to hospitals and other medical services as appropriate.

If your speciality is nursing, you can work in general practice nursing as a senior general practice nurse or an advanced nurse practitioner, and in district nursing as a senior district nurse/team leader or advanced community nurse practitioner. You’ll be employed in community-based primary care clinics, nurse managed health centres, community urgent and sub-acute care clinics, long-term care settings, community speciality clinics, public health departments, private medical practices, and work-based health clinics.
