This 20 credit module is offered at degree and masters level and is one of a range of specialised modules offered by the University. 

It contains a number of themes aimed at enhancing the understanding of key issues that influence the quality of care for all people with long-term conditions, whatever those conditions may be. These themes are:

  • person-centred care
  • self-management
  • service development.

You will focus on applying knowledge and understanding to support person-centred service development for people with long-term conditions and at masters level will also promote self-management and collaborative working.

ADV605 course details (degree level)

At degree level, this module will enable you to understand the need for service development, how to enhance person-centered care during routine practice and will teach you how to facilitate individuals with long-term conditions to maximise their own health and well-being through effective self-management. 
It is based upon current best practice within national and international policy and guidelines and will explore the impact of long-term conditions on the individual, their family and carers.

ADV735 course details (masters level)

At masters level, this module focuses on applying knowledge and understanding to support person-centred service development for people with long-term conditions and culminates in the submission of a service development proposal. You will be encouraged to further develop your knowledge and skills in order to maximise person-centred care during routine practice and develop services to support individuals with long-term conditions in line with health and social care policy.
You will focus on critically applying knowledge and understanding to support person-centred service development for people with long-term conditions. You will explore in depth the impact of long-term conditions on the individual, their family and carers based on best practice discussed within current national and international policy and guidelines.

ADV605 learning outcomes (degree level)

On successful completion of this module you will be able to:
  • demonstrate a systematic understanding of current Government policies and priorities in relation to long term conditions
  • critically evaluate models of service delivery
  • consider the application of health psychology to the management of long-term conditions in every day practice with a focus on self-management
  • explore and evaluate the evidence base relating to intervention programmes and service delivery as applied to your own area of practice.
Assessment
The assessment for this module is a 2,000 word written assignment evaluating a service currently being delivered.

The degree-level module can be taken as a standalone professional development module, as a core module towards our Professional Development in Long Term Conditions or as an optional module towards some other Professional Development in Health and Social Care pathways – please contact the Professional Development Unit for further information. 

ADV735 learning outcomes (masters level)

On successful completion of this module you will be able to:
  • critically analyse, discuss and debate government policies and priorities in relation to long-term conditions
  • critically appraise models of service delivery
  • critically analyse and discuss the application of health psychology to the management of long-term conditions with a focus on self-management
  • explore and evaluate the evidence base relating to intervention programmes as applied to your own area of practice.
Assessment
The assessment for this module is a detailed 4,000 word proposal on a new initiative in service design and delivery.

Advancing the management of long term conditions – distance learning ADV605Z / ADV735Z

Module dates
Dates for 2023/24 to follow soon. 
It is possible to undertake these modules via distance learning. Module content remains the same as the taught version but will be delivered by webinars, interactive teaching resources, Skype tutorials and other digital tools.

Entry requirements

To be considered for a place on this module:
  • you must be a registered health and social care professional or a health and social care worker in possession of previous study at level 5 (Diploma of Higher Education) or a foundation degree to study the module at degree level, or
  • you must be a registered health and social care professional or a health and social care worker who has previously studied to graduate level to study the module at masters level.
